Volunteer Agreement:By submitting this form, I affirm that the information provided is true and complete to the best of my knowledge. I understand that volunteering with the Zholistic Life Foundation may involve physical activity, interaction with the public, and participation in community environments. I agree to serve in good faith, with respect for others, and in alignment with the mission and values of the organization.
I acknowledge that all volunteer service is offered freely and without expectation of wages, benefits, or other compensation. I understand that I am not an employee of the Zholistic Life Foundation.
Release of Liability:I fully understand and accept that participation in volunteer activities may carry certain risks, including but not limited to physical injury, illness, property damage, or other unforeseen events. By signing or submitting this form, I voluntarily assume all such risks and hereby release, discharge, and hold harmless the Zholistic Life Foundation, its officers, directors, employees, representatives, and affiliates from any and all claims, liabilities, demands, or causes of action arising from or related to my participation as a volunteer.
This release includes, without limitation, claims for negligence (other than gross negligence or willful misconduct) and applies to any activity connected with my volunteer service, whether occurring on or off Foundation property.
Photo & Media Release:I grant permission for the Zholistic Life Foundation to use photographs, video recordings, or written testimonials of my participation for promotional, educational, or informational purposes without expectation of compensation.
Organizational Status Acknowledgement:I understand that the Zholistic Life Foundation is a nonprofit organization currently in its organizing and launch phase, and that all volunteer participation supports the development of its future community programs.
